SUPPORT COORDINATION

When a person living with a disability requires support, particularly from Professional Support Workers or caregivers, they often rely on organisations who have expertise at providing that care. These organisations frequently also provide guidance for referrals to other supports as needed. This is known as Support Coordination and is an important component of providing people with disabilities their needed care.

At AUSCARE DISABILTY SUPPORT SERVICES we recognise that it can be challenging to navigate the NDIS and put together an individual plan. Our support coordinators assist you to navigate your plan and work with you to find the most appropriate service and provider.

You and your support coordinator work closely to identify needs, personalise supports and make sure you are matched with the right services. Along the way, we support you to develop the skills and confidence you need to independently manage your NDIS plan.

To ensure you are continually receiving the best support, we check in from time to time to assess whether goals are being achieved and whether any changes or adjustments are needed.

During our Support Coordination meetings, we will:
  1. Identify services that meet your individual needs.
  2. Coordinate service providers and support.
  3. Liaise with government agencies.
  4. Organise the manner and timing of supports, including service agreements and obtain quotes for services.
  5. Source suitable accommodation and respite options.
  6. Link you into work or other community activities.
  7. Coordinate health and medical specialists.
  8. Implement and monitor your NDIS plan or re-develop your goals prior to a plan review.

What does Support Coordination with AUSCARE DISABILTY AND SUPPORT SERVICES look like?

Implementing your Plan

The first step is to understand who you are, what is important to you, and what you need from your Support Providers. We will assist you to understand your NDIS Plan and Support Budgets. From reviewing your current supports and organising Service Agreements, to engaging new services, arranging assessments, and ultimately helping you get maximum benefit and goal attainment from your NDIS Plan; a Support Coordinator is your go to for all things NDIS Plans.

Donations Received

It is our role as your Support Coordinator to make sure you are happy with the services you receive. We will observe this with you throughout the duration of your NDIS Plan and assist you with making changes to services if needed. Support Coordinators are also responsible in reporting to the NDIS on how your Plan is working and if the right supports are in place to achieve your goals. We will do this periodically – after your plan is approved and then throughout the year when required.

Supported Homes

If your needs change at any time during the year and you require different supports, we will assist you to put together the appropriate review documentation for submission to the NDIS for consideration. Before the end of the Plan, we will meet and decide if your goals are still relevant to your needs. We will contact your Support Providers to gather any reports or supporting documentation that will assist in the review process, to inform the NDIS of your support needs

Support Coordination is a service that identifies, coordinates, and enables access to support that meet a person’s specific needs and preferences by working with them and on their behalf. According to the NDIS, there are three levels of Support Coordination that can be included in your support plan depending on your needs and the level of your disability:

Support Connection

To increase your ability to connect with informal, community, and other funded supports encouraging you to make full use out of your plan and achieve your goals.

Support Coordination

Working with you to identify your needs and help you understand, implement, and utilise your plan. You can work with a Support Coordinator to get the most out of your supports and increase your functional capabilities, maintain relationships, manage the delivery of services, live more independently, and participate in the community.

Specialist Support Coordination

A higher level of Support Coordination for people whose circumstances are more complex and require specialist support. A Specialist Support Coordinator will manage specific challenges in your environment.

The NDIS funds reasonable and necessary supports for people living with disabilities. Everyone’s needs, and subsequently their support plan, are different, so not everyone will be eligible for Support Coordination funding. If you feel you will need Support Coordination, it’s a good idea to highlight the reasons you may need it so you can present this during your planning meeting.
Reasons you may be eligible for Support Coordination due to your disability include communication difficulties, language barriers, complex support needs, or if your social situation or support network is limited. People who are unable to make their own informed choices may also be eligible.

There are three ranges of Support Coordination, depending on how much support you need:

LOW

One to two hours a month or around 24 hours a year

MEDIUM

Three to six hours a month or around 36 to 72 hours a year

HIGH

Seven to nine hours a month
To assess the level of need of a participant, your planner will use the “reasonable and necessary” criteria of the NDIS, with the ranges listed above used as guidelines. These ranges will give your Support Coordinator an idea of how much support you need, but if you feel the level of support is lacking, you can request for reclassification.

Your Support Coordinator helps you use your NDIS plan in the most effective way possible for you. They work together with you to find and coordinate service providers that will help you reach your goals and build skills to live more independently and stay connected in the community. Other things your Support Coordinator can help you with include, but are not limited to, the following:
⦁ Help you understand how the plan works and what is available to you.
⦁ Support you in making decisions and exercise choice and control over your plan.
⦁ Assist with negotiating with providers about what they can offer you to help you reach your goals and how much it will cost.
⦁ Assist with budgeting for each type of support required.
⦁ Coordinate supports and help you access your local community.
⦁ Arrange any assessments needed to qualify for funding.
⦁ Develop service agreements and bookings with your providers.
⦁ Write up reports for the NDIS about your progress and any changes required.
⦁ Help you to assess your goals and prepare for your plan review.

Support Coordination

Case Management is similar to Support Coordination as it links someone living with a disability with their service providers and organises supports. However, Support Coordination is different to case management in that it is more about capacity building and achieving your goals.

Having funding for Support Coordination does not reduce the funding available for other supports in your plan. The Support Coordination fund is in addition to other supports.
